At Bella Vida MedSpa, patient safety and compliance with medical regulations are our top priorities. To ensure we provide the highest standard of care, a Good Faith Exam is required prior to receiving any medical aesthetic treatments.
What is a Good Faith Exam?
A Good Faith Exam is a comprehensive medical evaluation conducted by a licensed healthcare provider to determine your eligibility for treatments. Policy Guidelines
- No medical aesthetic treatments will be performed by any member of our staff without completing a Good Faith Exam.
- This policy applies to all patients, whether they are new to Bella Vida MedSpa or returning for additional services.
- The exam must be renewed periodically, depending on the type of treatments you receive, to ensure ongoing safety and effectiveness.

Photography and Video Policy
At Bella Vida MedSpa, we require photographs and, in some cases, video images to be taken as part of your treatment plan. These images are necessary to ensure your safety, track your progress, and document results, while also protecting the clinic by maintaining accurate medical records.
- Photography and Video Requirement:
All patients receiving treatments will have photographs taken by Bella Vida MedSpa or its representatives. These images will become a part of the patient’s medical record and are vital for ensuring proper documentation of the treatment process. - Use of Images:
Photographs or video images may be used for educational purposes (medical teaching or training) or for marketing purposes (website, print, digital, or social media). However, patients may request that their images remain solely within their medical record and not be used externally. - Confidentiality and Anonymity:
Images used externally will not include any identifying information, such as names. While measures are taken to maintain anonymity, patients acknowledge the possibility of being recognized by others. - Aftercare Documentation:
For patients requesting that their images only be part of their medical records, this preference will be noted in their chart to ensure compliance. - Liability Release:
By receiving treatment and adhering to this policy, patients release Bella Vida MedSpa, its employees, and third parties from liability related to the use of images for educational or marketing purposes.

Cancellation, Rescheduling and No-Show Policy
To ensure that we can maintain a high level of service for all our clients, we kindly ask you to review and agree to our Cancellation Policy before proceeding:
- 48-Hour Notice: We require at least 48 hours’ notice for any appointment cancellations or modifications. This policy allows us to offer your appointment time to another client and maintains a higher availability of services for you and other clients.
- Cancellation Fee: If you cancel or modify your appointment with less than 48 hours’ notice or do not show up for your scheduled appointment, you will be charged a cancellation fee of $50.
Please note, we require a valid credit card to be placed on file at the time of booking to finalize your appointment.
We will make every effort to accommodate changes and cancellations to scheduled appointments. Cancellation 48 hours or more prior to your appointment will be given a full refund of any deposits made if requested.
Children Policy
At Bella Vida MedSpa, we strive to create a serene and relaxing environment for all our patients to enjoy. To ensure the best possible experience for everyone, we kindly ask that children not be brought to appointments.
This policy is in place for several important reasons:
- Safety and Supervision: MedSpa treatments often involve the use of specialized equipment and products that require a controlled environment. For safety reasons, it is essential that our staff and patients can focus entirely on the procedure at hand without the need to monitor children.
- Relaxation for All: Many of our treatments are designed to provide a peaceful, rejuvenating experience. We want every patient to enjoy their time without distractions.
We understand that finding childcare can sometimes be challenging. However, for the safety of your child and the comfort of all patients, we are unable to accommodate children during appointments. Bella Vida MedSpa is not responsible for the care or supervision of children brought to the spa, and we appreciate your understanding and cooperation in respecting this policy.
We will make exceptions for brief appointments at times (Good Faith Exams, Weight Loss appointments, follow up photos), but it is best if you are able to have someone to monitor your children.

Returns and Refunds Policy

Please note that all pricing is subject to change, and the prices on this invoice do not guarantee future pricing. Services that are pre-purchased must be redeemed within one year of purchase.
As part of our policies, we do not offer refunds for services provided. Product returns are only accepted within 30 days of purchase and must be unopened and in their original packaging. Opened or used products are not eligible for return. Additionally, services cannot be exchanged once purchased.
