Good Faith Exam Policy

At Bella Vida MedSpa, patient safety and compliance with medical regulations are our top priorities. To ensure we provide the highest standard of care, a Good Faith Exam is required prior to receiving any medical aesthetic treatments.

What is a Good Faith Exam?

A Good Faith Exam is a comprehensive medical evaluation conducted by a licensed healthcare provider to determine your eligibility for treatments. This exam includes a review of your medical history, current health status, and aesthetic goals to ensure the selected treatment is both safe and appropriate for you.

Why is a Good Faith Exam Required?

This exam is mandated by California state law and ensures that every treatment aligns with your unique health needs. It also provides an opportunity for you to discuss concerns, ask questions, and make informed decisions about your care.

Policy Guidelines
  • No medical aesthetic treatments will be performed by any member of our staff without completing a Good Faith Exam.
  • This policy applies to all patients, whether they are new to Bella Vida MedSpa or returning for additional services.
  • The exam must be renewed periodically, depending on the type of treatments you receive, to ensure ongoing safety and effectiveness.

Photography and Video Policy

At Bella Vida MedSpa, we require photographs and, in some cases, video images to be taken as part of your treatment plan. These images are necessary to ensure your safety, track your progress, and document results, while also protecting the clinic by maintaining accurate medical records.

  1. Photography and Video Requirement:
    All patients receiving treatments will have photographs taken by Bella Vida MedSpa or its representatives. These images will become a part of the patient’s medical record and are vital for ensuring proper documentation of the treatment process.

  2. Use of Images:
    Photographs or video images may be used for educational purposes (medical teaching or training) or for marketing purposes (website, print, digital, or social media). However, patients may request that their images remain solely within their medical record and not be used externally.

  3. Confidentiality and Anonymity:
    Images used externally will not include any identifying information, such as names. While measures are taken to maintain anonymity, patients acknowledge the possibility of being recognized by others.

  4. Aftercare Documentation:
    For patients requesting that their images only be part of their medical records, this preference will be noted in their chart to ensure compliance.

  5. Liability Release:
    By receiving treatment and adhering to this policy, patients release Bella Vida MedSpa, its employees, and third parties from liability related to the use of images for educational or marketing purposes.

Cancellation, Rescheduling and No-Show Policy

To ensure that we can maintain a high level of service for all our clients, we kindly ask you to review and agree to our Cancellation Policy before proceeding:

  1. 48-Hour Notice: We require at least 48 hours’ notice for any appointment cancellations or modifications. This policy allows us to offer your appointment time to another client and maintains a higher availability of services for you and other clients.
  2. Cancellation Fee: If you cancel or modify your appointment with less than 48 hours’ notice or do not show up for your scheduled appointment, you will be charged a cancellation fee of $50.

Please note, we require a valid credit card to be placed on file at the time of booking to finalize your appointment.

We will make every effort to accommodate changes and cancellations to scheduled appointments. Cancellation 48 hours or more prior to your appointment will be given a full refund of any deposits made if requested.

Children Policy

At Bella Vida MedSpa, we strive to create a serene and relaxing environment for all our patients to enjoy. To ensure the best possible experience for everyone, we kindly ask that children not be brought to appointments.

This policy is in place for several important reasons:

  • Safety and Supervision: MedSpa treatments often involve the use of specialized equipment and products that require a controlled environment. For safety reasons, it is essential that our staff and patients can focus entirely on the procedure at hand without the need to monitor children.
  • Relaxation for All: Many of our treatments are designed to provide a peaceful, rejuvenating experience. We want every patient to enjoy their time without distractions.

We understand that finding childcare can sometimes be challenging. However, for the safety of your child and the comfort of all patients, we are unable to accommodate children during appointments. Bella Vida MedSpa is not responsible for the care or supervision of children brought to the spa, and we appreciate your understanding and cooperation in respecting this policy.

We will make exceptions for brief appointments at times (Good Faith Exams, Weight Loss appointments, follow up photos), but it is best if you are able to have someone to monitor your children.

Returns and Refunds Policy

Thank you for choosing Bella Vida MedSpa for your aesthetic and wellness care. We appreciate your trust in our services and are committed to providing high-quality, medically guided treatments.

Please review the following policies carefully:

  • All sales are final. Bella Vida MedSpa does not offer refunds for services rendered, prepaid services, treatment packages, memberships, or gift card purchases.

  • Prepaid services, treatment packages, and memberships are reserved at the time of purchase and are non-refundable once any portion of the treatment course has been initiated, regardless of whether all services have been redeemed.

  • Services are non-transferable and may not be exchanged once purchased.

  • Prepaid services must be redeemed within one (1) year of the original purchase date unless otherwise stated in writing.

  • Treatment outcomes and aesthetic responses vary by individual. Dissatisfaction with results, perceived lack of improvement, or subjective aesthetic concerns do not constitute grounds for a refund.

  • Clinical concerns or potential adverse effects require an in-person evaluation. Determinations regarding outcomes or next steps cannot be made based on photographs or remote communication alone.

  • Product returns are accepted within thirty (30) days of purchase only if unopened, unused, and in original packaging. Opened or used products are not eligible for return.

Pricing is subject to change at any time and prior pricing does not guarantee future pricing.

By purchasing services or products from Bella Vida MedSpa, clients acknowledge and agree to these policies.

Appointment Deposit Policy

To reserve certain appointments at Bella Vida MedSpa, a non-refundable deposit may be required at the time of booking. Deposit amounts vary by service and appointment length and will be disclosed at the time of scheduling.

Deposits will be applied toward the cost of the scheduled service on the day of treatment.

Deposits will be forfeited under the following circumstances:

  • Cancellation or rescheduling within 48 hours of the scheduled appointment

  • Failure to attend the appointment (no-show)

  • Choosing not to proceed with the scheduled service at the time of the appointment

Deposits are non-refundable and non-transferable and are required to reserve provider time and treatment resources. Exceptions are not made for scheduling conflicts, travel distance, or personal circumstances.

By booking an appointment that requires a deposit, clients acknowledge and agree to this policy.

Late Arrival Policy

To ensure timely care for all clients, we kindly request that you arrive on time for your scheduled appointment.

Clients arriving more than 10–15 minutes late (at the discretion of the provider and based on the scheduled service) may have their appointment:

  • Shortened to accommodate the remaining schedule, or

  • Rescheduled to a later date

If rescheduling is required due to late arrival, the appointment may be subject to the applicable cancellation or deposit policy. Late arrival does not guarantee additional time beyond the originally scheduled appointment duration.

Results Variability / No Guaranteed Outcome Policy

Aesthetic and wellness treatments involve biological processes that vary between individuals. As such, results are not guaranteed.

Treatment outcomes may be influenced by factors including, but not limited to:

  • Age, genetics, and skin type

  • Medical history and lifestyle factors

  • Compliance with pre- and post-treatment instructions

Dissatisfaction with results, perceived lack of improvement, or subjective aesthetic concerns do not constitute grounds for refunds, chargebacks, or complimentary treatments.

In-Person Evaluation Requirement Policy

Clinical concerns, treatment outcomes, and potential adverse effects require in-person evaluation by a qualified provider.

Assessments, diagnoses, or treatment decisions cannot be reliably made based on photographs, videos, phone calls, emails, or other remote communication alone. Bella Vida MedSpa reserves the right to require an in-office examination prior to determining appropriate next steps or modifications to care.

Consent & Treatment Plan Modification Policy

Treatment plans are developed collaboratively based on clinical assessment, patient goals, and professional judgment. However, treatment plans may be modified at the discretion of the provider on the day of service to ensure safety, appropriateness, and optimal outcomes.

Clients retain the right to decline or discontinue treatment at any time. Declining treatment may be subject to the applicable deposit or cancellation policy, as provider time and resources have been reserved.

Pre-Treatment Compliance Policy

Certain treatments require adherence to specific pre-treatment instructions to ensure safety and effectiveness. These instructions may include, but are not limited to:

•Sun avoidance

•Medication restrictions

•Hydration requirements

•Skincare product discontinuation

Failure to comply with pre-treatment instructions may result in:

•Treatment modification

•Treatment postponement

•Cancellation of the appointment

Such changes may be subject to the applicable deposit or cancellation policy.

Package Expiration Policy

Prepaid treatment packages must be redeemed within two (2) years of the original purchase date unless otherwise stated in writing.

Unused services remaining after the expiration period are forfeited and have no cash value. Extensions may be granted at the sole discretion of Bella Vida MedSpa and are not guaranteed.

Transferability Policy

All services, treatment packages, memberships, and prepaid appointments are non-transferable and may only be used by the original purchaser unless explicitly authorized in writing by Bella Vida MedSpa.

Sharing, gifting, or transferring services between individuals is not permitted.

Provider Substitution Policy

While we strive to accommodate provider preferences, Bella Vida MedSpa does not guarantee treatment by a specific provider.

Appointments may be performed by another qualified and appropriately trained provider when necessary due to scheduling changes, illness, or operational needs. Such substitutions do not constitute grounds for refunds or rescheduling without policy application.

Right to Refuse or Discontinue Treatment Policy

Bella Vida MedSpa reserves the right to refuse, postpone, or discontinue treatment at any time if deemed medically inappropriate, unsafe, or not in the client’s best interest.

This includes, but is not limited to:

•Safety concerns

•Non-compliance with policies or instructions

•Inappropriate behavior or conduct

In such cases, Bella Vida MedSpa is not obligated to provide alternative treatment or refunds beyond what is required by law or stated policy.

Chargeback & Payment Dispute Policy

Bella Vida MedSpa maintains detailed records of services rendered, appointments scheduled, consent forms, policies acknowledged, and communications exchanged.

By purchasing services or products, clients agree to contact Bella Vida MedSpa directly to attempt resolution of any billing concerns prior to initiating a chargeback or payment dispute.

Unauthorized chargebacks or disputes for services rendered, prepaid services, deposits, or policy-governed forfeitures may be contested with the payment processor using documentation including, but not limited to:

  • Signed consents and policy acknowledgments

  • Appointment records

  • Clinical documentation

  • Communication history

Initiating a chargeback does not override agreed-upon policies and may result in suspension of future booking privileges until the dispute is resolved.

Reviews & Public Commentary Policy

Bella Vida MedSpa values patient feedback and encourages direct communication regarding any concerns.

We ask that clients contact our office directly to address questions or dissatisfaction so that we may respond appropriately within a clinical and professional setting. Due to patient privacy laws and ethical standards, Bella Vida MedSpa is unable to discuss individual medical details or treatment specifics in public forums, including online reviews or social media platforms.

Public statements that contain false, misleading, or defamatory information may be addressed through appropriate channels. Posting a public review does not obligate Bella Vida MedSpa to alter clinical decisions, provide refunds, or deviate from established policies.

Adverse Event & Follow-Up Policy

While Bella Vida MedSpa utilizes evidence-based treatments and adheres to strict safety protocols, all medical and aesthetic procedures carry inherent risks.

Clients experiencing unexpected symptoms, adverse reactions, or concerns following treatment are required to notify Bella Vida MedSpa promptly. Appropriate follow-up care, evaluation, or management will be determined based on clinical assessment and may require an in-person visit.

Failure to follow post-treatment instructions or delay in reporting concerns may affect outcomes and limit available interventions. Follow-up recommendations are provided at the discretion of the treating provider and do not imply fault or guarantee a specific outcome.

Membership Modification & Termination Policy

Bella Vida MedSpa reserves the right to modify, suspend, or discontinue membership programs, pricing structures, or benefits with reasonable notice.

Memberships are non-transferable and non-refundable unless otherwise stated in writing. Bella Vida MedSpa may terminate a membership at its discretion for reasons including, but not limited to:

•Policy non-compliance

•Repeated appointment no-shows or late cancellations

•Inappropriate conduct toward staff or other clients

In the event of membership termination by Bella Vida MedSpa, unused benefits will be handled in accordance with the specific membership terms and applicable law. Termination does not entitle the member to refunds beyond what is expressly stated.
